Not necessary. Just intimate to the PAN Issuing centre alongwith a duly filled-in application form with the required fee, thereon.

Yes, its always best to file an FIR. You might get more answers at the income tax dept site -

Its better to file an FIR so that you are not responsible if your PAN card is being misused. Also, contact the Income Tax dept to know how to apply for a new PAn card.
