One degree higher and at the very least the person will suffer brain damage and possibly death. Any higher and this usually results in death for the person, however, there have been records of people reaching 44 degrees and surviving.

When you are ill, you are infected either by bacteria or virus. In order to compensate the damage done by these organisms, body tries to burn more ATPs which is the source of energy. In this process there is lot of heat is generated and hence the tempearture increases during fever.

The temp comes from the infection.
As for rising at night, maybe blankets, maybe baths, maybe not eating or drinking fluids...Lots of factors. But in general it shouldn't be much different, if at all, during night and day.
