Yes , he is considered out as the middle stump is off the ground if not the bails.
In cricket the rule is subsides with either the bails or stumps are to be removed before the batsmen so in case of being bowled by a bowler if a stump is uprooted instead of bails, still he is considered out.
In bowled or run out either the bails or stumps have to off in this as the middle stump is uprooted the batsman is considered out, if the stumps wasn’t uprooted from the ground and just moved with bails still on the top he cannot be given out as nor the bails or stumps are off.
