u hav to visit the government gazzete ofiice with ur legal name,thus after taking the nominal govt.fees the name is announced in the leading newspaper for the name change & latter the name is changed to the new name in the govt. gazete

writer has given the exact answer to your question...you would have to fill up a form that is available on the india govt web site and follow the procedure....if you are a minor..signatures of parents and guardian is a must...after all the formalities have been done and your name is changed...publicise that in atleast two daily newspapers....you would also have to change your name in your passport and banks where you hold an account...

u must go to the gazette publications and apply for a change of name, after paying a nominal amt. After that u must quote this gazette no and notify this changed names in any two popular dailies. Then , u must keep a copy of the gazette magazine that arrives with the published version of ur new name. Only if u produce this gazette copy, will ur new name be accepted.
